The new forms may be downloaded from this website, under Application Forms. BOTH parents must sign it and have it notarized, unless ONE parent has legal guardianship. All of our front gate people are now Notary Publics, meaning that the form may be taken care of at the gate (your parents or guardians must be with you for the signing).
